Job summary

SUN MAR HEALTH CARE is seeking a licensed Physical Therapist Assistant to provide therapy services in post-acute settings. Work under the supervision of a PT, document patient progress, and coordinate with the care team in Vineland post-acute facilities.

Ideal candidates have 1 year PTA experience, CPR certification, and licensure per state guidelines. Part-time positions with varied shifts are currently available.

Qualifications

• Minimum of Associate of Arts in Physical Therapy Assistant.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Physical Therapy Assistant experience preferred.
• 1-2 years in residential/assisted living or skilled nursing facility preferred.

Responsibilities

• Provides Therapy services established by the Physical Therapist (PT) in the patient Plan of Care.
• Provides therapy services under the supervision of a PT as determined by the Registered Therapist and in compliance with State and Federal guidelines.
• Consults with PT regarding changes in treatment or condition in a timely manner.
• Completes documentation including daily notes and assists PT to create progress notes, discharge summaries, or monthly reports.
• Submits billing, payroll, and expense documentation in a timely manner.
• Communicates with patients, families, referring physicians and other team members.
• Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
• Participates in care planning meetings, rehab meetings and family training to communicate patient progress.
• Follows organizational and facility policies and procedures to ensure safety.
• Maintains licensure/certification as requested.

• Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
• Coordinates treatment goals with other team members.
• Maintains valid licensure, certification or equivalent as requested by appropriate state agency.

Requirements

Experience

• 1 year Physical Therapy Assistant experience preferred.
• 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
• Graduates in Physical Therapy Assistant program will be considered.
