Job summary

Fowler Care Center seeks a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the supervision of a licensed Physical Therapist, the PTA helps patients recover through targeted therapy focused on strength, mobility and functional independence in a skilled nursing setting.

The ideal candidate holds a California PTA license, a PTA degree from an accredited program, and CPR/BLS certification. SNF rehab experience is preferred, but new grads are welcome to apply.

Qualifications

  • CA PTA license and degree from an accredited PTA program required.
  • CPR/BLS certification must be current.
  • Experience in SNF, rehab, or post-acute care preferred; new grads welcome.

Responsibilities

  • Assist PT in implementing treatment plans to improve strength, mobility and function.
  • Provide skilled interventions per the PT plan of care.
  • Document patient progress and communicate changes to the supervising PT.
  • Educate patients and families on exercises, mobility and fall prevention.
  • Maintain a safe, clean treatment area and participate in team meetings.

Physical Therapist Assistant (PTA)
Position Description

Fowler Care Center is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the direction and supervision of a licensed Physical Therapist, the PTA provides skilled therapy services to help patients recover from illness, injury, surgery, and other medical conditions. This role focuses on improving strength, mobility, balance, gait, and overall functional independence while delivering high-quality, patient-centered care in a collaborative skilled nursing environment.

Qualifications
  • Graduate of an accredited Physical Therapist Assistant (PTA) program.

  • Current and unrestricted California Physical Therapist Assistant (PTA) license.

  • Current CPR/BLS certification.

  • Skilled Nursing Facility (SNF), rehabilitation, or post-acute care experience preferred; new graduates are welcome to apply.

  • Knowledge of physical therapy principles, rehabilitation techniques, therapeutic exercise programs, and functional mobility training.

  • Strong clinical observation, documentation, communication, and organizational skills.

  • Ability to work collaboratively with Physical Therapists, physicians, nursing staff, and the interdisciplinary team.

  • Compassionate, dependable, and committed to providing exceptional patient-centered care.

  • Ability to safely perform the physical demands of the position, including assisting with transfers, ambulation, and therapeutic activities.

Responsibilities
  • Assist the Physical Therapist in implementing individualized treatment plans designed to improve strength, flexibility, balance, gait, endurance, mobility, and functional independence.

  • Provide skilled physical therapy interventions in accordance with the Physical Therapist's plan of care.

  • Treat patients recovering from orthopedic injuries, joint replacements, neurological conditions, and other illnesses affecting mobility and function.

  • Observe, document, and communicate patient progress, treatment responses, and changes in condition to the supervising Physical Therapist.

  • Complete accurate and timely documentation in compliance with Medicare guidelines, facility policies, and state and federal regulations.

  • Educate patients and families on therapeutic exercises, mobility techniques, fall prevention, and safe use of assistive devices.

  • Collaborate with nursing, therapy, physicians, and other interdisciplinary team members to promote optimal patient outcomes.

  • Maintain a safe, clean, and organized treatment environment while ensuring therapy equipment is used appropriately.

  • Participate in interdisciplinary meetings, care conferences, and quality improvement initiatives as assigned.

  • Maintain compliance with HIPAA, infection prevention standards, and all applicable federal, state, and facility regulations.

  • Perform other duties as assigned by the Physical Therapist or Rehabilitation Director.
