Trinity Healthcare Talent Services, LLC is seeking a highly skilled Physical Therapist Assistant to join our team at a skilled nursing facility in Lebanon, New Hampshire. You will work with our Physical Therapist to deliver high-quality care and rehabilitation services to residents.

This role provides 40 hours per week, Monday–Friday with weekend rotation, and offers a competitive hourly salary range of $29–$34 per hour.

Qualifications

• Associate's degree in Physical Therapy Assistant program from an accredited institution.
• Current state license or eligibility for state licensure as a Physical Therapist Assistant.
• Minimum of 1 year of experience in a skilled nursing facility or similar setting preferred.
• Strong knowledge of physical therapy principles and techniques.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient‑centered approach to care.

Responsibilities

• Assist the Physical Therapist in evaluating and developing individualized treatment plans for patients
• Implement treatment plans and provide hands‑on therapy to patients under the supervision of the Physical Therapist
• Monitor and document patient progress and adjust treatment plans as needed
• Educate patients and their families on exercises and activities to improve their mobility and function
• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients
• Maintain accurate and up-to-date patient records and documentation
• Adhere to all state and federal regulations and ethical standards in patient care
