Flagship Therapy in Jacksonville, FL is seeking a Physical Therapist Assistant for our in-house rehab program. This full-time position supports geriatric patients with comprehensive rehabilitation plans and wound care when applicable.

The PTA screens and evaluates patients, delivers skilled treatment, documents progress, and prepares discharge summaries while meeting regulatory standards. New grads welcome; PTA license required.

Qualifications

• Physical Therapist Assistant license is required.
• Open to PTAs at all experience levels. New Grads are welcome!

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver skilled and medically necessary treatments.
• Assist with patient transfers and mobility as needed.
• Administer procedures as part of rehab plan (manual therapies, exercises, modalities).
• Provide counseling to patients and families; coordinate with other providers.
• Generate discharge summaries and ensure regulatory compliance.

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Cedar Hill Post-Acute and Rehab in Jacksonville, FL. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking dedicated Full Time Physical Therapist Assistant for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
