Rehab Center of the Rockies in Fort Collins, CO is seeking a PRN Physical Therapist Assistant to join our in-house rehab program. The PTA will screen, evaluate, and implement rehabilitative plans for geriatric patients in a post-acute setting, with emphasis on wound care and mobility.

Open to PTAs at all experience levels; new grads are welcome. This role supports a stable, in-house therapy team under CAPLICO valued standards and promotes patient-centered care in a post-acute environment.

Qualifications

• PTA license is required.
• Open to PTAs at all experience levels; New Grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders; develop treatment plans.
• Deliver rehabilitative interventions per plan and regulatory standards.
• Assist with patient transfers, standing, walking, and wound care as needed.
• Provide patient, family, and caregiver counseling about physical disorders.
• Prepare discharge summaries in compliance with regulatory requirements.

Join our team and start making a difference! Rehab Center of the Rockies in Fort Collins, CO, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN Physical Therapist Assistant for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
