Job summary

Elara Caring is seeking a Physical Therapist Assistant to deliver home health services in Cleveland’s east side, Euclid, and surrounding communities. You’ll work under a Physical Therapist to help patients regain mobility and independence in daily activities, visiting patient homes across the territory.

Requirements include graduation from an accredited PTA program, an Ohio PTA license, and a willingness to travel ~50%. Home health experience and Homecare Homebase familiarity are aPlus.

Qualifications

• Graduation from an accredited PTA program.
• Current, unrestricted PTA license valid in Ohio.
• 6 months or more of PTA experience preferred.
• Willingness to travel approximately 50%.
• Ability to frequently lift up to 50 pounds.
• Valid driver’s license or reliable transportation.
• Home health/homecare Homebase experience preferred.

Responsibilities

• Provide physical therapy services according to the Plan of Care established by the supervising Physical Therapist.
• Review the care plan before each visit and incorporate patient goals into treatment.
• Monitor patient response and promptly report changes to the Physical Therapist.
• Teach patients and caregivers how to follow their home therapy program.
• Instruct patients in the safe use of walkers, canes, wheelchairs, and other prescribed assistive devices.
• Document care accurately and follow applicable practice standards and Ohio regulations.
• Collaborate with the care team to support quality and continuity of care.

Getting out of a favorite chair. Walking to the kitchen. Managing the front steps. As a Physical Therapist Assistant with Elara Caring, you’ll help patients work toward goals that matter in their daily lives.

We’re seeking a PTA to serve Cleveland's east side, Euclid, and surrounding communities. You’ll provide treatment under a Physical Therapist’s direction, build relationships with patients and caregivers, and help patients gain strength, mobility, and confidence at home.

What You’ll Need

• Ability and willingness to travel throughout the assigned territory, with approximately 50% travel.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
