Grove Park Healthcare Center is actively seeking a Physical Therapist Assistant (PTA) to join our dedicated rehab team. Under the supervision of a Physical Therapist, you will help implement treatment plans and support residents on their rehabilitation journeys.

The role emphasizes accurate documentation in EMR systems, patient education, and collaboration with an interdisciplinary team. A valid PTA license and CPR/BLS certification are required.

Qualifications

• Licensed Physical Therapist Assistant in the state (PTA).
• Experience with EMR documentation and patient data collection.
• CPR/BLS certification current.
• Knowledge of data collection requirements (Section GG).
• Strong clinical and interpersonal skills.

Responsibilities

• Assist with assessments and carry out treatment plans under supervision.
• Implement treatment plans, exercises, modalities and mobility training.
• Monitor residents’ progress and report changes to the supervising PT.
• Maintain accurate and timely documentation in EMR (Net Health/PCC).
• Educate residents and caregivers on safe mobility techniques.
• Collaborate with the rehab team under guidance of the Physical Therapist.

Physical Therapist Assistant, PTA Job Description:

The Physical Therapy Assistant (PTA) works under the supervision of the Physical Therapist to implement treatment plans and assist residents in their rehabilitation journey.

Physical Therapist Assistant, PTA Responsibilities:

• Assist in Assessments: Assist the Physical Therapist in carrying out treatment plan and gathering objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
• Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
• Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (P O S) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.

Physical Therapist Assistant, PTA Qualifications:

• State licensure as a Physical Therapy Assistant.
• EMR and MS office applications.
• CPR/BLS certification.
• Knowledge of Section GG data collection and documentation requirements.
• Strong clinical and interpersonal skills.
