Remedy Home Health Care is hiring a Physical Therapist Assistant (PTA) for the East LA Territory, covering Downey area. The role focuses on rehabilitation, fall prevention, and functional mobility, with one-on-one in-home patient care under a licensed Physical Therapist.

You will deliver skilled therapy, educate families, and collaborate with the care team to support senior patients in their homes, with per diem flexibility and opportunities for growth.

Qualifications

• Active California Physical Therapist Assistant license required.
• Current CPR certification (AHA) required.
• Minimum 1 year of clinical experience; home health preferred.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Qualifications:

• Active California Physical Therapist Assistant (PTA) license
• Current CPR certification (AHA required)
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation
