Remedy Home Health Care in Orange County is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team on a per diem basis. You will deliver one-on-one therapy in patients' homes to help regain mobility and independence.

The role offers flexible scheduling, autonomy to manage your own patient roster, and the chance to contribute to senior care in the community. You will work under a licensed Physical Therapist and document using WellSky Kinnser EMR.

Qualifications

• Active California PTA license
• Current CPR certification (AHA)
• Minimum 1 year of clinical experience (home health preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation

Responsibilities

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.
