Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their therapy services. The PT is an essential healthcare provider committed to helping patients reach their highest functional level. The therapist will uphold the mission, vision, and values of Jefferson Health.
They will collaborate with other healthcare professionals to coordinate treatment plans and care.

*The Amputation Team is seeking a dedicated and compassionate Physical Therapist to join as an Amputation Rehabilitation Specialist in the outpatient setting.

This is a full-time, fixed position on the Amputation Team, managing a caseload of outpatient amputation patients. The role includes supervision and mentorship from experienced team members and offers opportunities for professional development.

Job Description

Essential Functions:

  1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full caseload across acute care, skilled nursing facilities, acute rehab, and outpatient settings as appropriate.
  2. Participate in student education for part-time (ICE, OT level I) or full-time students.
  3. Assist in orientation and preceptorship of new or rotating staff.
  4. Engage in education, teaching, and professional activities related to the position.
  5. Design, organize, and implement treatment programs to aid recovery and rehabilitation.
  6. Use appropriate tools and techniques to ensure safe therapeutic interventions and training for patients and families.
  7. Document clients' health progress, including evaluations, treatments, progress notes, and discharge summaries.
  8. Perform other duties as assigned.
