Job Description

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The PT is an integral healthcare provider accountable for helping patients progress to their highest level of function. The therapist will uphold the mission, vision, and values of Jefferson Health and will work in conjunction with other healthcare professionals to coordinate treatment plans and care.

Essential Functions:

  1. Interacts with co-workers, visitors, and staff consistent with Jefferson's values.
  2. Provides therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload in various settings such as acute care, skilled nursing facilities, rehab, and outpatient clinics.
  3. Participates in student education, including ICE, OT level I, or full-time students.
  4. Assesses, synthesizes, and applies literature to clinical practice.
  5. Participates in education and teaching suitable for novice clinicians.
  6. Plans, organizes, and delivers treatment programs to aid patient recovery and rehabilitation.
  7. Utilizes tools and techniques to administer safe therapeutic interventions and training for patients and families.
  8. Records and documents health progress, interventions, evaluations, and discharge summaries.
  9. Assists with staff orientation, mentorship, and competency assessments.
  10. Suggests and implements program development or performance improvement activities with supervisor approval.
  11. Completes educational, teaching, and professional activities as appropriate.
  12. Supports departmental operations and performs other duties as assigned.

Education: Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.

Experience: Minimum of _ years of current clinical practice.

Licensing and Certifications: State license required; BLS and CPR certifications required.
