Summary

We are hiring for a Physical Therapist with a 7 on / 7 off schedule. Part-time or PRN positions also available!

• Traditional Schedule (Monday through Friday primary)
• 7 On / 7 Off

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patients' needs related to mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate patients' functional status and therapy needs, assisting physicians in developing care plans.
• Monitor and report patient responses to treatment and any condition changes to the healthcare team.
• Treat patients to relieve pain, restore function, and maintain maximum functioning according to standards of practice.
• Educate patients, families, and healthcare team members about the care plan, limitations, and progress.

Education and Experience

Licensure Requirements:

• Current Physical Therapy license in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
