Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this assessment, the therapist helps develop a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

  • Evaluates the patient's functional status and physical therapy needs. Assists the physician in developing the therapy plan of care.
  • Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treats patients to relieve pain, develop and restore function, and maintain maximum functioning according to accepted standards of practice.
  • Instructs and informs healthcare team members, patients, and families/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Education and Experience

Licensure Requirements:

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
