Qualifications

  • Active Pennsylvania Physical Therapist License.
  • Home health experience preferred.
  • BLS certification.
  • Valid driver’s license and reliable transportation.
  • Current car insurance.
  • Ability to travel throughout Northeast Philadelphia and Montgomery County service areas.

  • Provide physical therapy evaluations, treatments, and follow-up visits in patients’ homes.
  • Develop individualized treatment plans based on patient needs and goals.
  • Perform mobility assessments and therapeutic interventions.
  • Educate patients and caregivers on exercises, safety techniques, and home programs.
  • Document patient progress and care plans accurately and timely.
  • Collaborate with healthcare providers and interdisciplinary teams to support patient outcomes.
  • Maintain compliance with home health documentation and quality standards.

Location: Northeast Philadelphia & Montgomery County, PA

We are seeking a dedicated and experienced Physical Therapist to provide high-quality home health care services throughout Northeast Philadelphia and Montgomery County, PA. This is a great opportunity for a PT who enjoys working independently while helping patients achieve their mobility and recovery goals in the comfort of their own homes.

  • Location: Northeast Philadelphia & Montgomery County, PA
  • Service Areas: 19141 and 19120 zip codes
  • Schedule: Monday – Friday, 8:00 AM – 4:30 PM
  • Hours: 40 hours per week
  • Assignment Duration: 13 weeks
  • Pay Rate: Up to $83.70/hour
