Job summary

Rehab Without Walls Neuro Rehabilitation in Ann Arbor, MI, seeks an experienced Physical Therapist to provide functional, home-based therapy. Join a team delivering care beyond the clinic using real-world settings to improve daily living activities.

You will design individualized plans, reassess progress, and collaborate with a multidisciplinary team to support patients in cooking, shopping, work tasks, and community activities. A flexible schedule and growth opportunities are available.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in practicing state.
• BLS or CPR certification per state regulations.
• At least 1 year of physical therapy experience.
• Knowledge of rehabilitation techniques for complex neurological injuries.
• Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
• Physical stamina: standing, walking, bending, reaching, and lifting.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Regularly reassess treatment plan effectiveness; attend staffing meetings and family conferences.
• Conduct discharge evaluations and recommend home and community safety modifications; provide referrals and educational resources.
• Communicate patient needs and progress to the interdisciplinary team, physicians, patients, and families.
• Collaborate across disciplines to coordinate care and improve patient outcomes.
