Responsibilities
- Perform patient evaluations and develop individualized treatment plans.
- Regularly reassess treatment plan effectiveness; attend staffing meetings and family conferences.
- Conduct discharge evaluations, recommend home and community safety modifications, and provide referrals and educational resources.
- Communicate patient needs and progress to the interdisciplinary team, physicians, patients, and families.
- Collaborate across disciplines to coordinate care and improve patient outcomes.
Qualifications
- Degree in Physical Therapy from an accredited program.
- Active Physical Therapist license in practicing state.
- Certification in Basic Life Support or CPR as per state regulations.
- At least one year of physical therapy experience is preferred.
- Knowledge of rehabilitation techniques for complex neurological injuries.
- Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
- Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.
- Health screenings or exams may be required per location or state policies.
