Job summary

Loving Care In-Home Health Services in Yukon, Oklahoma seeks a licensed Physical Therapist to provide high-quality home health physical therapy. Part-time position performing initial evaluations, treatment planning, and delivery of care in patients' homes under physician guidelines.

Qualifications

• Must be licensed as a Physical Therapist in Oklahoma.

Responsibilities

• Perform initial patient evaluations and provide routine Physical Therapy services per physician plan of care.
• Assess and treat patients using physical therapy modalities.
• Formulate and execute treatment plans based on patient needs.
• Instruct patients and family in home-based care.
• Ensure timely documentation within required timeframes (evaluation and visit notes).
• Collaborate with office staff, physicians, and other therapists.

Responsibilities and Duties

• Performs initial patient evaluations and provides routine Physical Therapy services to patients according to prescribed plan of treatment by a physician.
• Assess and treat patients with physical therapy through use of physical modalities.
• Formulates treatment plans based upon individual patient need.
• Assist persons who are physically challenged to improve their function and mobility and daily living activities.
• Instructs patients and family teaching in the home.
• Initiates appropriate discharge planning when necessary.
• Participates in weekly patient case conference meetings and any staff meetings as notified.
• Communicates with office staff, physicians, other therapists, etc.
• Serves as a resource to agency staff regarding physical care of patients.
• All patient evaluation documentation shall be completed no later than 48 hours from time of completion of evaluation.
• All care visit notes to be completed within 24 hours from time of completion of visits.
• WellSky software used for all patient EMRs.
• Participates in any Performance Improvement Projects and Quality Improvement as needed.

Qualifications

• Must be currently licensed in the State of Oklahoma as a Physical Therapist
• Minimum of 1 year of clinical and/or professional experience as a licensed Physical Therapist.
• Medicare Home Health experience is preferred.
