Job summary

Premier Home Health Providers Inc. is seeking a per diem Physical Therapist to assess and treat patients in their homes across California.

You will evaluate functional status, mobility, strength, gait, and ROM, and develop individualized plans of care in collaboration with physicians. Responsibilities include delivering PT services per plan of care, educating patients and families, and coordinating with the care team.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.
