BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) for part-time or per diem home health visits. You will assess clients in their homes, design personalized plans, and collaborate with a care team to improve strength, balance, mobility and safety.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred.

Responsabilités

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
