Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home physical therapy on a part-time or per diem basis. You will evaluate patients, develop personalized care plans, and deliver interventions to improve strength, balance, mobility, and safety in the home.

We offer flexible scheduling, mileage reimbursement, and a supportive team with a mobile EMR for easy documentation. Active MA PT license and CPR certification are required; new grads welcome.

Qualifications

• Active MA PT license and CPR certification required.
• DPT or MPT from an accredited program is required.
• 1+ year of experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create care plans.
• Provide skilled PT interventions to improve strength, balance and mobility.
• Educate clients and families on exercises and safety at home.
• Collaborate with multidisciplinary care team and document in EMR.
