Caring Hand Home Health Solutions, Inc is seeking a Physical Therapist to provide skilled services in patients' homes across California. This per diem role requires an active California PT license and a minimum of 2 years of PT experience; home health experience is a plus.

You will evaluate mobility, gait, and ROM, develop treatment plans, educate patients and families, and collaborate with physicians and the care team.

Qualifikationen

• Bachelor's or Master's in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years PT experience; home health background a plus.
• Current CPR certification (AHA).
• Valid driver's license and reliable vehicle.

Aufgaben

• Evaluate patients' functional status, mobility, muscle strength, gait and ROM to create individualized plans.
• Deliver PT services per physician's plan of care and California PT Practice Act.
• Educate patients and families on exercises, equipment use and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PTAs and oversee Home Health Aide care bi-weekly when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team - reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver's license and reliable, insured vehicle.
