Job summary

Caring Hand Home Health Solutions, Inc. is seeking a licensed Physical Therapist to deliver skilled home health services in California. The role focuses on evaluating and treating patients, restoring mobility, and supporting independence in day-to-day activities.

Per diem position offering pay per visit ($160-$180) with potential for full-time conversion based on performance. Travel to patients' homes, collaboration with care teams, and adherence to California PT statutes are required.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health experience preferred.
• Strong communication skills with patients and families.
• Current CPR certification with AHA; valid driver's license and insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
