Caring Hand Home Health Solutions, Inc. is looking for a committed Physical Therapist to join their team in Vallejo, California. In this per diem role, you will evaluate and treat patients at home, helping them regain mobility and confidence.

The ideal candidate will have a degree in Physical Therapy and at least 2 years of experience. Benefits include flexible scheduling, skills training, and the ability to convert to full-time based on performance.

Formación

• Minimum 2 years of PT experience; home health background is a plus.
• Active California Physical Therapy license is required.
• Current CPR certification with AHA is essential.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

Requirements

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
