Job summary

Angels Care Home Health is seeking a Physical Therapist to assess and treat patients at home, designing and executing care plans to restore or maintain wellbeing. You will plan, organize, and oversee physical therapy treatments based on medical referrals and evaluations, while coordinating with an interdisciplinary team.

Ideal candidates hold a current Physical Therapist license, with at least two years of PT experience, plus CPR and Hepatitis profiles.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plan, implement, and evaluate patient care plans to restore or maintain well-being.
• Plan, organize, and conduct PT treatment programs based on referrals and evaluations.
• Collaborate with interdisciplinary team to coordinate patient care.
