Job summary

Angels Care Home Health is seeking a Physical Therapist to plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.

Collaborates with interdisciplinary team. Qualified candidates have a current state Physical Therapist license, at least two years of experience, current CPR and Hepatitis profile, and knowledge of durable medical equipment.
