Job summary

Jobtailor in Wilmette, IL is seeking a Physical Therapist to provide rehabilitative services to residents in a senior living community as part of a coordinated care team.

You will plan and deliver individualized treatment programs based on clinical findings, participate in clinical conferences, and conduct one-on-one sessions to support functional-based care throughout the care plan.

Qualifications

• Degree required in an area of discipline.
