Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our growing team in Eastern Ohio, including Dayton. You will evaluate, develop, and implement individualized treatment plans to help residents improve mobility, reduce pain, and maintain independence in assisted and independent living settings.

Qualifications

• Active State PT license in Ohio.
• DPT or equivalent degree.
• CPR/BLS certification required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled PT treatments including therapeutic exercise, gait training, balance training, transfers, and mobility interventions.
• Reassess progress and modify treatment plans accordingly.
• Supervise PTAs in line with state regulations.
• Document care timely in EMR system.
• Educate residents, caregivers, and staff on plans, home programs, and fall prevention.
• Collaborate with interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload while delivering quality therapy.

About the Role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
