AdvisaCare is seeking a licensed Physical Therapist to provide home health therapy in Michigan. You will evaluate, plan, and implement personalized treatment to improve mobility and independence for clients in their homes.

The role emphasizes compassionate, patient-centered care, collaboration with an interdisciplinary team, and documentation of progress. A valid Michigan license and driver's license are required, with autonomy and reliable transportation to patient locations.

Qualifications

• Licensed Physical Therapist with Michigan license.
• Minimum 1 year home health care preferred.
• Strong communication and patient engagement skills.
• Self-motivated with ability to work autonomously.
• Valid driver’s license and reliable vehicle for travel to patient locations.

Responsibilities

• Develop and implement individualized treatment plans focused on mobility, strength, balance, and function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
• Educate patients and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to standards.
