Job summary

Enhance Rehabilitation in Illinois is seeking qualified Physical Therapists to join our team in SNF settings. As a Physical Therapist, you will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to ensure high-quality care, supervise Physical Therapist Assistants, re-evaluate progress, adjust plans, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Licensed Physical Therapist with SNF experience preferred.
• Ability to develop plans of care and set functional goals.
• Strong communication and patient education skills.
