Job summary

Solvara Partner Services, Inc. is seeking a Physical Therapist to join our home-care team in Waterbury, CT and surrounding communities.

You’ll evaluate patients in their homes, develop individualized treatment plans, and help improve mobility, balance, and safety with a focus on independence. We offer part-time and per diem positions with flexible scheduling and the potential to transition to full-time as caseload grows.

Qualifications

• Active Connecticut Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Ability to work independently in a home-care setting.
• Strong clinical assessment and communication skills.
• Reliable transportation.
• Previous home-care experience preferred, but not required.

Responsibilities

• Perform physical therapy evaluations and reassessments in patients' homes.
• Develop individualized treatment plans.
• Provide skilled therapy addressing strength, balance, gait, mobility, transfers, and fall prevention.
• Educate patients and caregivers on safety and home exercise programs.
• Complete timely and accurate clinical documentation.
• Communicate with other healthcare professionals involved in patient care.

Solvara Partner Services, Inc. is a locally owned, therapist-operated rehabilitation company looking for Physical Therapists to join our growing home-care team in Waterbury and surrounding communities.

We're currently hiring part-time and per diem therapists, with strong potential for a quick transition into a full-time salaried position as our caseload grows.

We're not a large corporate home-health organization. We're practicing therapists who understand what it's like to manage patient visits, travel between homes, complete documentation, and still have time for life outside of work.

At Solvara, you'll work directly with the owners. No regional managers, no unnecessary layers of administration, and no complicated chain of command. We believe in giving therapists the flexibility, independence, and professional respect they deserve.

We're expanding our coverage in the Greater Waterbury area and are looking for therapists interested in growing with us.

While we're hiring for part-time and per diem positions now, we see strong potential for these opportunities to develop into full-time salaried roles as our caseload expands.

You don't need to cover every town. We're especially interested in therapists who already live or work in the area and would like to build a caseload closer to home.
