Job summary

NorthLakes Community Clinic is seeking a Physical Therapist to assess and plan patient-centered therapy aimed at improving mobility and function. You will develop and document comprehensive plans of care, educate patients on exercises and adaptive equipment, and provide hands-on therapy as needed.

Candidates should have a Master’s in Physical Therapy, 1–2 years of experience, and an active Wisconsin license. EMR experience is preferred; residency within service area is required where applicable.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• 1–2 years of experience in a similar role.
• Active Wisconsin Physical Therapy license; NPTEx exam passed.
• EMR experience preferred; tech literacy beneficial.

Responsibilities

• Evaluate the patient to determine goals and needs.
• Diagnose movement dysfunction and plan care.
• Create plans of care and document patient records per requirements.
• Educate patients on therapeutic exercises and adaptive equipment.
• Provide hands-on therapy as applicable.
• Collaborate with other providers to optimize outcomes.
• Maintain a clean and safe work environment and equipment.
• Perform additional duties as assigned.

Responsible for planning and administering physical therapy treatment plans designed to promote overall fitness and health of patients.

Will consider part-time or full-time candidates

Essential Job Functions

Essential Functions of the Job: [Other duties may be assigned.]

• Consult with and evaluate the patient to determine patient goals and needs.

• Diagnose and treat movement dysfunction.

• Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.

• Provide education on therapy exercise techniques and use of adaptive equipment such as walkers or wheelchairs.

• Provide hands‑on therapy such as massage, thermotherapy, dry needling, etc. as applicable.

• Work collaboratively with other providers including other therapists, surgeons, and the patient’s primary care provider.

• Clean and maintain equipment to ensure clean and safe work environment.

• Other duties as assigned.

Qualifications and Education Requirements

• Minimum Level of Completed Education: Masters Degree in Physical Therapy from an accredited physical therapy school

• Prior years of experience in similar role: 1-2

• Experience with systems, computer applications/software: None required. Electronic Medical Record experience preferred.

• Required (or Preferred) Licensure/Cert/Credentials: Successful passing of the National Physical Therapy Examination; must have active Wisconsin Physical Therapy license
