Job summary

NorthLakes Community Clinic is seeking a qualified Physical Therapist to plan and administer treatment plans that promote overall fitness and health for patients in our service area. Candidates must reside in the service area and be eligible to work within Wisconsin licensing standards.

The role includes diagnosing movement dysfunction, creating care plans, documenting records, and collaborating with a team of providers. Part-time to full-time opportunities are available.

Qualifications

  • Master's degree in Physical Therapy from an accredited program.
  • 1–2 years of experience in a similar role.
  • Successful passing of the National Physical Therapy Examination; active Wisconsin Physical Therapy license

Responsibilities

  • Consult with and evaluate the patient to determine goals and needs.
  • Diagnose and treat movement dysfunction.
  • Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.
  • Provide education on therapy exercise techniques and use of adaptive equipment.
  • Provide hands-on therapy such as massage, thermotherapy, dry needling, etc. as applicable.
  • Work collaboratively with other providers including other therapists, surgeons, and the patient's primary care provider.
  • Clean and maintain equipment to ensure clean and safe work environment.
  • Other duties as assigned.

Essential Job Functions

  • Consult with and evaluate the patient to determine patient goals and needs.

  • Diagnose and treat movement dysfunction.

  • Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.

  • Provide education on therapy exercise techniques and use of adaptive equipment such as walkers or wheelchairs.

  • Provide hands-on therapy such as massage, thermotherapy, dry needling, etc. as applicable.

  • Work collaboratively with other providers including other therapists, surgeons, and the patient's primary care provider.

  • Clean and maintain equipment to ensure clean and safe work environment.

  • Other duties as assigned.

Qualifications and Education Requirements

  • Minimum Level of Completed Education: Masters Degree in Physical Therapy from an accredited physical therapy school

  • Prior years of experience in similar role: 1-2

  • Experience with systems, computer applications/software: None required. Electronic Medical Record experience preferred.

  • Required (or Preferred) Licensure/Cert/Credentials: Successful passing of the National Physical Therapy Examination; must have active Wisconsin Physical Therapy license
