Job summary

Sapphire Nursing at Wappingers in New York is seeking a caring, dedicated Per Diem Physical Therapist to provide quality care to its residents. The therapist will evaluate and treat patients, communicate with families and health team members, and document services in medical records.

The PT will supervise assistants, develop rehabilitation plans, and help residents improve mobility while maintaining confidentiality and complying with state and federal requirements.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Nursing Home Experience preferred.

Responsibilities

Job description

Sapphire Nursing at Wappingers, is seeking a caring, dedicated Per Diem Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient's medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients' physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

PT Duties Include:

• Supervise the activities of physical therapy assistants.
• Evaluate Residents conditions & develop appropriate physical rehabilitation plans.
• Help Residents improve their mobility.
• Relieve Resident's pain using exercises, hands-on therapy & stretching.
• Teach Residents how to use physical therapy equipment.
• Help Residents through their tasks and monitor their performance.
• Maintain Resident confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Submit all billing logs and necessary information on a timely basis.

PT Requirements Include:

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Nursing Home Experience preferred
