Job summary

Fishkill Home Care in New York is seeking a caring and dedicated Physical Therapist to provide high-quality care to its patients. The PT evaluates and treats patients, communicates with families, physicians, and the health team, and maintains all necessary documentation in the patient’s medical records.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current New York State PT license.
• Nursing home experience preferred.

Responsibilities

• Supervise the activities of physical therapy assistants.
• Evaluate patient conditions and develop rehabilitation plans.
• Help patients improve their mobility and manage pain.
• Discharge planning and coordination with rehab team.
• Maintain patient records and confidentiality.
• Submit billing logs and necessary information in a timely manner.

Fishkill Home Careis seekinga caring and dedicated Physical Therapists to provide quality care to its Patients. Our Physical Therapists evaluates and treats Patients, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient’s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients’ physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program.

PT Duties Include:

• Supervise the activities of physical therapy assistants.
• Evaluate Patients conditions & develop appropriate physical rehabilitation plans.
• Help Patients improve their mobility.
• Relieve PatientsResident's pain using exercises, hands-on therapy & stretching.
• Teach Patients how to use physical therapy equipment.
• Help Patients through their tasks and monitor their performance.
• Maintain Patient confidence & protects the facility by keeping information confidential.
• Complies with federal, state, and local legal and professional requirements
• Contributes to team effort.
• Supervisors PTA according to State and Agency Policy.
• Submit all billing logs and necessary information on a timely basis.

PT Requirements Include:

• Graduate of an accredited Physical Therapy Program
