Job summary

VitalCaring is hiring a Home Health Physical Therapist to serve patients at home throughout Clearwater, FL. You will deliver patient-centered care, evaluate functional status, and develop individualized plans in collaboration with the care team.

In this highly autonomous role, you will monitor progress, educate patients and caregivers, and coordinate care with nurses, therapists, and physicians to ensure seamless transitions and positive outcomes.

Qualifications

• Must hold a current physical therapy license in Florida.
• 1–2 years of home health experience preferred.
• Excellent communication and patient education abilities.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Coordinate with nurses, therapists, and physicians to ensure coordinated care and discharge planning.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certificationValid driver’s license, reliable transportation, and auto insurance
