Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings. You will evaluate functional status, design individualized plans, and work with physicians and the care team to restore mobility and independence for patients in Florida.

This highly autonomous role offers professional growth opportunities and the chance to make a meaningful difference by bringing high-quality therapy directly to patients in their homes.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or MS/MSPT) and licensed in FL.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment, critical thinking and patient education skills.
• Excellent communication and CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Evaluate and treat patients in home settings to improve mobility and independence.
• Develop and implement individualized care plans with physicians and care team.
• Monitor progress, adjust plans, and document care in EMR.
• Educate patients and caregivers on exercises and safety.

What You’ll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certificationValid driver’s license, reliable transportation, and auto insurance
