We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Sumter, SC area. If you have a passion for providing patient care, improving outcomes, and building community relationships, Your Health Org is the perfect opportunity for you.

Physical Therapist role performs visits in homes, ALF's, and ILF's, clinics, and via Telehealth. This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 12-hour position, 7 am-7 pm, 7 days on & 7 days off.

What Are We Looking For?

Your Health is seeking a compassionate and empathetic Physical Therapist to join our growing primary care team. The ideal candidate will provide the highest standards of care, possess excellent communication and interpersonal skills, and be able to interact positively and motivatingly with patients. Responsibilities include obtaining health and psychosocial histories, performing physical assessments, providing health education and counseling, arranging referrals, and ensuring confidentiality and proper documentation. Community education and outreach are also part of the role.

Qualifications

• Successful completion of a Physical Therapy training program and certification through the state board
• Current license to practice in SC
• Current CPR certification
• Previous healthcare experience preferred
• Ability to work flexible hours and travel between locations
• Proficiency in electronic documentation
• At least one year of experience in the relevant field
• Interest or experience in Pelvic Health (preferred)
