We are seeking a compassionate Physical Therapist to join our team! The ideal candidate has a positive attitude, excellent interpersonal skills, and a current state PT license. You will provide medical support to patients while ensuring excellent customer service.

Responsibilities

• Provide physical therapy services according to a physician's plan of care, including assessment, treatment, and patient education.
• Evaluate patients' therapeutic, rehabilitative, and functional status, and participate in developing the care plan.
• Assess muscle strength, mobility, gait, and range of motion to determine rehab potential.
• Direct physical therapy treatments and instruct patients and caregivers on the use of therapeutic devices.
• Report patient progress and reactions to physicians and participate in case conferences.
• Train patients in prosthetic device use if applicable.
• Identify additional home health needs and refer patients to appropriate services.
• Prepare and submit clinical and progress summaries, participate in discharge planning, and provide consultation and education as needed.
• Supervise physical therapy assistants and, when applicable, home health aides.

Qualifications

• Degree from an accredited physical therapy program.
• State licensure as a physical therapist.
• Minimum of two years' experience, preferably in community or home health settings.
• Strong communication and organizational skills.
• Current CPR certification.
