Job summary

Osborn Health in Scottsdale, AZ, is seeking a full-time Physical Therapist to join our in-house rehab team. This role offers a stable schedule and a focus on geriatric care within a post-acute setting.

Licensed PT with all experience levels considered. You will screen patients, implement plans of care, and collaborate with a multidisciplinary team to improve cognitive and ADL function and outcomes.

Qualifications

• Licensed Physical Therapist in good standing.
• Open to PTs at all experience levels.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL impairments and develop and follow plans of care.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled treatment and consult with patients and families as needed.
• Administer procedures as part of rehabilitation plans including ADL retraining and therapeutic exercises.
• Monitor patient progress and adjust treatments in clinical records.

PHYSICAL THERAPIST - FULL TIME - OSBORN HEALTH - SCOTTSDALE, AZ

Osborn Health in Scottsdale, AZ, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Pay

$43 - $53/hour based on experience

Key Responsibilities

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
