BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist for part-time/per diem, providing in-home evaluations and personalized care plans. You’ll help clients regain strength, balance, and independence in their own homes.

The role offers a flexible schedule, reimbursement for mileage, and a supportive team environment with EMR documentation and hands-on leadership.

Qualifications

• Active MA PT license
• DPT or MPT from an accredited program
• CPR certified
• 1+ year of experience preferred (new grads welcome)

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
