BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to perform in-home evaluations and deliver personalized care plans in patients' homes.

Part-time or per diem positions offer flexible scheduling, mileage reimbursement, and opportunities to contribute to patient independence and safety with a supportive team and mobile EMR documentation.

Qualifications

• Active MA PT license.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
