Job summary

Marin Post Acute in Marin County, CA is seeking a part-time or per diem Physical Therapist (PT) to join our rehabilitation team. Compensation is $65-$70 per hour, with flexible scheduling to support work-life balance and resident needs.

The ideal candidate holds a DPT or equivalent, is licensed in California, and brings strong clinical assessment and communication skills to deliver patient-centered care in post-acute and long-term care settings.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to post-acute, rehabilitation, and long-term care residents
• Collaborate with physicians, nursing staff, therapy professionals, and families to ensure optimal outcomes
• Monitor progress and adjust treatment plans based on resident needs
• Educate residents and caregivers on mobility, safety, strengthening, and functional independence
• Maintain accurate and timely documentation in compliance with regulatory and reimbursement requirements
• Participate in interdisciplinary care planning and discharge planning activities

At Marin Post Acute, we are committed to delivering exceptional rehabilitation services that help residents regain strength, mobility, and confidence. We are currently seeking compassionate and motivated Physical Therapists (PTs) to join our therapy team on a Part-Time or Per Diem basis.

What You'll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to post-acute, rehabilitation, and long-term care residents
• Collaborate with physicians, nursing staff, therapy professionals, and families to ensure optimal outcomes

Preferred:

• Skilled Nursing Facility, Post-Acute, Rehabilitation, or Long-Term Care experience
• Familiarity with Medicare documentation and reimbursement guidelines
• Experience treating orthopedic, neurological, cardiac, and medically complex patients
