Job summary

Collabera in San Jose, CA seeks a Physical Therapist for a full-time direct-hire role. You will evaluate patients, develop treatment plans, and deliver PT services while collaborating with an interdisciplinary team. The ideal candidate holds an active PT license with CPR, and a DPT or master's in PT, plus strong orthopedic/neurological/geriatric competencies. Proficient EHR use and excellent communication are required.

Qualifications

• Active state licensure as a Physical Therapist and CPR certification.
• Doctorate (DPT) or master's degree in physical therapy from an accredited program.
• Strong clinical skills with orthopedic, neurological, or geriatric PT experience.
• Excellent communication and interpersonal skills, with ability to work independently and with a team.
• Proficient computer skills for accurate electronic documentation and patient records.

Responsibilities

• Provide PT services and evaluate patients to develop treatment plans.
• Implement and adjust therapy plans collaboratively within an interdisciplinary team.
• Document accurately in EHR and maintain patient records in a timely manner.
• Monitor patient progress and modify interventions as needed.
• Ensure compliance with licensure and professional standards.

Position: Physical Therapist
Location: San Jose, CA - preferred location
Other Locations: Morgan Hill, Brawley, Calexico, El Centro, Concord, Castro Valley, Stockton, Caldwell, ID, Star, ID/ Idaho Falls, ID/Reno, NV/Fort Worth, TX/Arlington, WA/Burien, WA - 5 day onsite
Duration: Full-Time Employment
Pay Range: $80k - 115k per annum

• Active state licensure as a Physical Therapist, along with CPR certification (and any state-required credentials).
• Doctorate (DPT) or master's degree in physical therapy from an accredited program.
• Strong clinical skills with a background or hands-on experience in orthopedic, neurological, or geriatric physical therapy.
• Excellent communication and interpersonal skills, with the ability to work independently and collaborate effectively within an interdisciplinary team.
• Proficient computer skills for managing accurate, timely electronic documentation and patient record-keeping.
