Health Link Home Health & Hospice in San Francisco is seeking a Physical Therapist (PT) for home health care. You will evaluate and treat patients in their homes, develop care plans, and supervise PTAs as needed.

The role emphasizes mobility, safety, and caregiver education, with AI-assisted documentation tools to streamline charting and ensure a consistent, high-quality patient experience. A $10,000 sign-on bonus is offered.

Qualifikationen

• Home health experience is preferred. Strong outpatient, SNF, or hospital PTs are encouraged to apply and we’ll provide home health orientation and OASIS training.
• Valid California driver’s license is required.
• Ability to travel regularly within San Francisco and surrounding areas.

$64.90–$91.35/hour (approx. $135K–$190K annually full-time) | $10,000 Sign-On Bonus
