Job summary

DayToDay Health is seeking a licensed Physical Therapist to provide comprehensive PT services, including assessment, treatment planning, and interventions in an interdisciplinary setting. You will evaluate patient needs, develop individualized goals, document progress, and coordinate care with nurses and physicians, while supervising Physical Therapy Assistants.

This role requires a Doctorate in Physical Therapy, active licensure, BLS, a valid driver's license, and the ability to perform

Qualifications

• Doctorate in Physical Therapy (required after 2015) or equivalent.
• Licensed Physical Therapist with current credentials and BLS certification.
• Valid driver’s license and auto liability insurance.
• Strong decision-making, time management, and communication skills.
• Ability to perform physical tasks including lifting up to 50 pounds.

Role Overview

The Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting.

Key Responsibilities

• Perform treatments such as muscle strengthening, stretching, heat/cold therapy, gait training, and ADL training based on physician prescriptions.
• Evaluate patient needs and develop individualized treatment plans with measurable goals.
• Document patient progress, communicate with healthcare team, and coordinate care with nurses and physicians.
• Supervise Physical Therapy Assistants and Home Health Aids.
• Contribute to program development, quality improvement, and departmental policies.
• Maintain supplies, equipment, and ensure safety and infection control protocols.

Qualifications & Skills

• Doctorate in Physical Therapy (required after 2015) or equivalent.
• Licensed Physical Therapist, BLS certification, valid driver’s license, and auto liability insurance.
• Strong decision-making, time management, and communication skills.
• Ability to perform physical activities, including lifting up to 50 pounds, and adapt to various working conditions.
• Proficiency in electronic documentation and resource management.

Physical & Working Conditions

Must have adequate strength, mobility, and stamina to assist patients and perform physical activities. Work environment is primarily well-lit and climate-controlled, with exposure to infectious agents and potential emergencies.
