Job summary

Covenant Village seeks a licensed Physical Therapist to assess, plan, and deliver physical therapy services according to physician orders and the North Carolina Physical Therapy Practice Act. You will provide 1-on-1 rehabilitation at Covenant Village and in patient homes across Rowan and Mecklenburg Counties.

You will evaluate movement, develop tailored care plans, educate patients and families, and collaborate with multidisciplinary teams.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current Physical Therapy license in North Carolina, or eligibility to obtain it before the start date.
• Excellent clinical, interpersonal, and fiscal management skills.
• Proficiency with computers and electronic documentation systems.
• Experienced, motivated, and compassionate Physical Therapist.

Responsibilities

• Provide individualized, 1-on-1 rehabilitation treatments at Covenant Village and in patient homes throughout Rowan and Mecklenburg Counties.
• Evaluate home environments, muscle strength, gait, mobility, and range-of-motion potential.
• Develop tailored rehabilitative care plans.
• Educate patients, families, and caregivers on home exercises, safe mobility strategies, and therapeutic appliances.
• Participate in clinical conferences.
• Coordinate integrated care plans with multidisciplinary teams.
• Identify referrals for additional home health (PCA) services as needed.
• Maintain progress summaries based on goal attainment.
• Supervise Physical Therapist Assistants (PTAs) in compliance with state regulations and company standards.
