Job summary

Jobtailor in Gastonia, NC seeks a licensed Physical Therapist to assess and treat patients under physician orders, delivering 1-on-1 rehabilitation in homes and Covenant Village.

You will develop personalized care plans, educate patients and families, participate in clinical conferences, and collaborate with multidisciplinary teams while ensuring state compliance and high-quality care.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current North Carolina Physical Therapy license, or eligibility to obtain it before the start date.

• Proficiency with computers and electronic documentation systems.

Responsibilities

• Assess, plan, and deliver physical therapy services according to physician orders and the State Physical Therapy Practice Act.
• Provide individualized 1-on-1 rehabilitation treatments at Covenant Village and in patient homes throughout Gaston County.
• Evaluate home environments, muscle strength, gait, mobility, and range-of-motion potential.
• Build tailored rehabilitative care plans.
• Educate patients, families, and caregivers on home exercises, safe mobility strategies, and therapeutic appliances.
• Participate in clinical conferences.
• Coordinate integrated care plans with multidisciplinary teams.
• Identify referrals for additional home health PCA services.
• Maintain progress summaries based on goal attainment.
• Supervise Physical Therapist Assistants in compliance with state regulations and company standards.
