Responsibilities:

1. Assist the Physical Therapist in carrying out treatment plans and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
2. Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
3. Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
4. Documentation: Maintain accurate and timely documentation of resident progress and interventions, including Point of Service (PoS) care delivery and documentation, resident evaluations, treatment plans, daily and progress notes, and HMO/Manage care updates in EMRs (Net Health/PCC).
5. Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-ins through Remote Therapeutic Monitoring (RTM) with discharged patients at home or next level of care.
6. Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and cooperating with other rehab staff.
