CareOne is seeking a Physical Therapist to join its in-house rehab team in Randolph, MA. You will collaborate daily with nursing and clinical staff to improve patient outcomes, ensuring continuity and stability as part of our team across a large multi-state network.

The role emphasizes clinical excellence and professional growth. You will evaluate residents, develop and adjust treatment plans, document thoroughly, and may travel to other facilities.

Qualifications

• Master's or Doctorate in Physical Therapy and an active PT license.
• Experience with older adult rehabilitation preferred.
• Knowledge of healthcare regulations in care settings preferred.

Responsibilities

• Provide and direct resident rehabilitation care per standards.
• Evaluate resident needs and establish treatment plans.
• Coordinate supervision of treatment plans with staff.
• Screen residents and contribute to clinical documentation.
• Provide feedback to supervision on rehabilitation issues.
• Participate in all center processes and services.
• Maintain billing records to regulatory standards.
• Travel to other facilities to provide services as needed.

The Physical Therapist's (PT) overall responsibility is to provide care to the resident at the highest level.

Position Information

Min $40/hr - Max $65/hr

Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, and work schedule

Location: Randolph, MA

Facility: CareOne at Randolph

Practice Setting: (Short Term Rehab, Assisted Living, LTACH, Long Term Care)

Essential Duties and Responsibilities

• Provides and directs resident/resident rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
• Evaluates residents/resident needs and establishes effective and efficient treatment plans for the resident/residents assigned.
• Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
• Participates in the screening of all residents according to directed schedules and practices.
• Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
• Participates in all center directed processes and services.
• Performs necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
• Maintains complete, accurate and timely billing records to meet center, state, federal, and accrediting agency requirements
• Participates in in-service training programs as requested
• May travel to other facilities to provide services on a limited basis

Position Requirements

• Master's or Doctorate Degree in Physical Therapy.
• Current and Active Physical Therapist license within the state of operations.
• Prefer-experience/knowledge in older adult rehabilitation.
• Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings.
