Job description

Central Health Management, Inc. is seeking a full time Physical Therapist for a Monday - Friday 8 hour shifts providing care for patients in their home in Southwest Virginia.

Provide physical therapy services to patients under appropriate guidelines.

· Implement and conduct treatment as outlined in treatment plan.

·Treats clients to relieve pain, develop and restore function andmaintain maximum performance using appropriate physical therapytreatments and procedures within the plan of care.

· Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.

· Interpret and communicate evaluation findings to patients, family and other staff members.

Qualifications:

• Active and Valid Physical Therapy Licensed in Virginia
• Valid Driver's license
• BLS/CPR certified
• Prefer Home Health experience, not required
